Spring 2022 Self-Management Programs

The following virtual classes are free and open to the public:

Virtual Diabetes Self-Management Program

In partnership with The Giving Fridge, Porter Medical Center will be hosting a free virtual Diabetes Self-Management program for people diagnosed with type 2 diabetes. Participants will learn how to better manage their symptoms by focusing on seven self-care behaviors: healthy eating, being active, monitoring blood sugar, taking medication, problem solving, healthy coping, and reducing risks.

Participants and immediate family members will be provided with free locally sourced meals courtesy of The Giving Fridge. Pick up will be available on Thursday afternoon’s, between 4-5:30 at 66 Merchants Row, Middlebury.

  • Beginning Wednesday, March 23, 4:00 – 6:30 pm

Virtual Chronic Pain Self-Management Program

In partnership with Springfield Medical Cares System, this program is for individuals living with long-term (or “chronic”) pain. Participants will learn how to make healthier eating choices, increase levels of physical activity with respect to the chronic pain they are dealing with, manage medications more safely, manage stress and work through emotional challenges that come with living with chronic pain.

  • Beginning Monday, April 11, 2:00 – 4:30 pm

Virtual High Blood Pressure Management

In partnership with Brattleboro Memorial Hospital, this program is for individuals who have been diagnosed with hypertension and who are interested in learning how to better manage their high blood pressure at home. Participants will learn the basics of controlling high blood pressure, learn how to monitor blood pressure at home, better manage blood pressure medications, reduce stress and stay motivated.
