August 2022 Self-Management Programs

The following virtual classes are free and open to the public:

Virtual Chronic Disease Self-Management Program

In partnership with North Star Health, Porter Medical Center will be hosting a free virtual Chronic Disease Self-Management program for people living with a long-term (or “chronic”) health problem—like arthritis, diabetes, cancer, obesity, or heart disease. Participants will learn how to better manage their chronic illness by making healthy lifestyle changes, which can help to lower the risk of serious health events down the road, like heart attack, stroke, breathing problems, memory loss, and falls.

Beginning Thursday, August 18th, 2022 I 12:00 – 2:30 pm

Virtual Quit Smoking Program

Porter Medical Center is offering a free virtual Quit Smoking program for individuals who smoke or use tobacco products. In this program, participants will create a plan of action for becoming tobacco free, learn how to stay motivated for the long run and find ways to manage stress and cravings. Vermont Quit Partners offer FREE gum, lozenges, and patches for those who participate.

Beginning Tuesday, August 23rd, 2002 I 12:00- 1:00 pm
