Do you make house calls?
Yes, insurance requires that people meet certain guidelines to be seen at home. As long as these guidelines are met, these individuals can be seen in their private residences, in a community care home, or in an assisted living facility.
Who are your patients?
Home-bound individuals with life-limiting illness who seek a shift away from intensive medical intervention. We deliver care which aims to minimize hospitalizations, testing, and medications that do not contribute to improved quality-of-life.
Do you offer 24 hour coverage?
Coverage will be provided 24 hours per day, 7 days per week to assure continuity and consistency of care
What community linkages do you offer?
We work closely with community care homes, Helen Porter Health Care and Rehabilitation Center, Addison County Home Health and Hospice, Elderly Services, Champlain Valley Agency on Aging and other social service agencies.
Are you affiliated with a hospital?
The practice has been established in conjunction with Porter Medical Center, Middlebury, VT.
Porter’s CEO, James L.
