About Us

Partners in Palliative and Home Care has been established in conjunction with Porter Medical Center. Porter’s CEO, James L. Daily, stated, “There is a growing recognition among hospital and hospice leaders that patients and families are in need of a more integrated continuum of care from their local delivery system, and that community healthcare providers have a responsibility to assure that such a continuum is available.”

“Partners in Palliative and Home Care” represents an innovative, patient-centered approach that focuses on the special needs of homebound patients with serious and chronic illnesses and their families. The overall goal is to allow the patient to live at home with as much space, dignity, and personal control as possible. “

“Providing collaborative medical care at Helen Porter Health Care and Rehabilitation Center is a necessary and vital component of the comprehensive philosophy of Partners in Palliative and Home Care. This ensures high quality “home care” to patients residing temporarily or permanently in the nursing home setting. This approach allows individuals and their families to expect and receive the best possible care in a variety of settings.”

—Leslie Orelup, FNP

“For the people in our community, expanded access to coordinated, high-quality palliative and home care can help facilitate greater personal autonomy and control over care and treatment choices. Our goal is to bring a higher degree of humanity and sensitivity to illness-related emotional, psychological, and spiritual issues that otherwise might get lost in the delivery of traditional medical treatment.
