We provide continuing, comprehensive health care for the individuals of all ages. Our practice includes acute and chronic care for all ages, male and female, each organ system and every disease entity. Primary Care – Brandon, located in Brandon, VT is part of the UVM Health Network – Porter Medical Center with practices located throughout Addison County and in Brandon.
We are a Certified Patient-Centered Medical Home under the Vermont Blueprint for Health, which means that we have been recognized by the National Committee for Quality Assurance for facilitating a partnership between our patients and providers to assure that patients get the care they need when and where they need it. The care team gives patients and family access to evidence-based care and self-management support.
Our overall goal is to provide access to high quality health care and excellent customer service to the people of our community through out team of professional staff, as well as via the additional resources of our Community Health Team members.
