RE:Community Health Survey
Porter Hospital, in concert with other
local health and human services agencies, will be working to develop a "Community Health Needs Assessment" for our hospital service area over the next few months. Part of this process is collecting input and opinions from members of our community regarding your views on our local health care system.
Below this memorandum is a two-page survey that we would ask you to complete and return to me c/o Porter Hospital, 115 Porter Drive, Middlebury, VT

Thank you for your time and your input. We will publish the results of this Community Health Needs Assessment by the end of 2004 in our first "Porter Hospital Report Card to the Community", and
